Prepare for Your First Visit

Once you have made your initial appointment, you may prepare for your appointment by simply downloading the forms, print them, fill them out and bring them with you to your first visit. Completing the forms prior to your visit will help to speed up the new patient registration process and get you in to see the doctor faster!

We look forward to helping you with your pain management needs.

Please note: You must have a responsible adult drive you to and from any procedure appointment. The driver will need to stay in the waiting room while you are receiving the procedure. If you do not bring a driver to your appointment, the procedure will be postponed and a $50.00 late cancellation fee will apply.
